See applications for due dates. For medical release forms, print out the form and make copies for each player. Bring the completed medical release forms and final roster to check-in the first day of the tournament. Do not send in rosters and medical release forms early. Further information will be sent in the form of a confirmation letter after the application and payment have been received.
